The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered as a detailed description of all the work requirements that may be inherent in the position


1. Follows current, established standards of nursing practice and implements facility policies and procedures.

2. Interprets existing policies and procedures to LPN s, CNA s, residents/patients and their families.

3. Directs resident/patient care and initiates corrective actions as necessary.

4. Obtains report from the nurse being relieved and records sufficient information to implement appropriate follow-up care.

5. Provides report to the nurse coming on duty.

6. Conducts resident/patient rounds daily, reporting problems to DON/NHA and initiates decided corrective action in a timely manner.

7. Identifies safety hazards and initiates corrective action immediately.

8. Follows, directs activities toward, evaluates and implements Resident s Rights.

9. Actively participates in resident/patient, facility education programs.

10. Provides supervision to LPN s and CNA s in direct resident care.

11. Attends staff meetings, interdisciplinary team meetings, rehabilitation meetings, in-service classes, committee meetings or other meetings, conferences and seminars, as assigned.

12. Makes nursing assignments according to qualifications of staff and characteristics of the resident/patient problems.

13. Assumes active responsibility for compliance with Federal, State and Local regulations.

14. Administers and accurately documents direct resident care, medications and treatment per

physicians orders in a timely manner.

15. Provides instruction, and/or teaches, appropriate nursing care to LPN s and CNA s according to current practice standards.

16. Perform comprehensive assessment of residents/patients and documents the result of those assessments in a timely and accurate manner.

17. Develops and implements an accurate, current and comprehensive assessment and initiates alternatives as necessary.

18. Promptly reports changes in resident s/patient s conditions to the physicians, DON/NHA and takes follow up action.

19. Follows facility admission procedures and communicates resident/patient admission to other departments, physicians and ancillary service providers.

20. Competently performs current practice nursing skills.

21. Follow facility discharge/transfer procedure and coordinates discharge/transfer with other departments, physicians and ancillary service providers. Completes documentation related to resident s/patient s discharge/transfer accurately and thoroughly.

22. Responds to resident/patient call for assistance in a timely manner.

23. Actively listens to resident/patient, family or physician concerns and initiates corrective action and/or reports concerns to DON/NHA.

24. Accurately counts and documents the narcotic count, exchanges keys, checks emergency kit, checks crash cart and consistently documents their status on the related forms.

25. Orders, receives and stores medications appropriately.

26. Notifies physician promptly of changes in resident/patient condition related to but not exclusive to weight, falls, injuries, medication errors, medication not available, abnormal lab/x-ray results and any other unusual observations. Documents in the nurse s notes that the physician was notified and what the physician response was to the concern.

27. Documents daily notes on unstable residents/patients, Medicare residents/patients, and recently admitted residents/patient.

28. Consistently works in a cooperative, respectful and courteous manner with residents/patients, other nursing staff, physicians families, consultants, community agencies, ancillary service providers and superiors.

29. Assumes responsibility for the safety of all residents/patients and consistently works toward a safe care environment.

30. Comes to work as scheduled and consistently demonstrates dependability and punctuality.

31. Performs all duties assigned in an effective, cooperative, timely and professional manner.

32. Observes infection control procedures.

33. Responsible for IV s, tube feedings, CVP lines, PICC lines and blood transfusions.

34. Assists with orientation of new personnel either directly or indirectly.

35. Makes team assignments and other assigned duties.

36. Completes age specific competencies.

37. Knowledge in regards to ADA s (American s with Disability Aspects).

38. Conducts performance improvement monitoring and evaluation activities in a thorough and accurate manner. Takes an active role in the PI process.

39. Keeps current in the geriatric current field of practice through knowledge and skill competencies.

40. Satisfactorily completes or keeps current certification as an ACLS provider.

41. Performs in depth, systematic assessments of residents/ patients.

42. Maintains an awareness of unit budget.

43. Functions as a professional role model and resource person serving as an orientation preceptor for new employees.

44. Complies with established chain of command for work related problems.

45. Demonstrates/models the Geriatric Center s core values and customer service behaviors in interactions with all customers (internal and external).

46. Maintains confidentiality of all material handled as well as proper release of information on individuals or events.

47. Collaborates to assure appropriate staffing levels on all shifts.

48. Functions as a team member of the Geriatric Center regardless of shift work.

49. Supports unit goals and objectives in a positive and cooperative manner.


  • Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.

Applicants will be evaluated through a pre-employment Physical Therapy Back School and Body Mechanics Screen Test. Below is a list of the specific tasks and the strength requirements necessary to perform the task.

Leg Lift 80lbs.

Ability to withstand high stress situations.


At risk for punctures, being confined to small spaces, slippery services, assault and battery, radiation, chemical and bio-hazards, injuries from operating machinery, burns and back injuries.

Employees who work in patient care areas where they may have potential exposure to patients with suspected or proven tuberculosis (TB) must have the ability to wear a particulate respirator and be fit-tested in compliance with the current recommendations from the CDC (enforced by OSHA) or must provide physician documentation as to the inability to wear a particulate respirator.:

ADDITIONAL REQUIREMENTS: Recent experience within the past two years in a long term care setting. Current working knowledge of DOH and JCAHO regulations in long term care, MDS, UMRL restorative nursing.




Graduate of a professional RN program.

Current Pennsylvania RN license.

Trained to provide service to patients of all ages from pediatric to geriatric.

Maintenance of CPR certification.


Working knowledge of all aspects of geriatric care.


  • Must be under the supervision of an experienced RN at all times.
  • Needs to successfully complete the State Board Examination within sixty days after graduation.


Day, middle or night shift with varying hours based on the needs of the department.
